ACKNOWLEDGEMENT AND CONSENT

I hereby acknowledge that I have read and understood clearly and completely the Privacy Notice of Taiyo Gases Co., Ltd. (the "Company"), and hereby consent to the collection, use, processing, storage, disclosure and/or transfer of my personal data in and out of Thailand by the Company in accordance with any and all of the provisions in and for any and all purposes set out in the Notice.
